Community Health Access and Navigation in Tennessee (CHANT)

  • We engage families through home visits, calls, and office visits.
  • We make sure you have the right tools and education to set up your family for success.
  • We strive to make sure you have access to local resources for whatever situation you may be going through.
  • We empower you to be your own advocate for your family.

Another part of CHANT is our Children & Youth with Special Health Care Needs (CYSHCN). CSS may provide comprehensive medical care for children with physical disabilities from birth to 21 years of age. Diagnostic and financial eligibility criteria must be met to participate in the program.

We host Monthly Parenting Class with various topics each month. They are an hour long and involve discussion and a hands on activity.
