Community Health Access and Navigation in Tennessee (CHANT)

You know what you need, but maybe you're not sure how to get it. That's where we come in. Whether it's applying for insurance, sorting out medical bills, finding doctors or dentists, or even finding employment or housing, we can connect you to services that will help in your situation.
Who is eligible?
- Pregnant and postpartum women
- Families with children up to age 21
Children & Youth with Special Health Care Needs
The CHANT program may provide comprehensive medical care for children with physical disabilities from birth to 21 years of age. Diagnostic and financial eligibility criteria must be met to participate in the program.
Referrals
CHANT referrals can be made by all medical providers and social service agencies. Self-referrals to CHANT are also accepted. The CHANT referral form may be accessed here.
